Two tubes, the same brand, the same £6.99 price, the same fluoride-free hydroxyapatite technology. If you have stood in front of Biorepair Plus Sensitive Teeth and Biorepair Plus Total Protection wondering whether the difference is real or just packaging, this article is the answer. The short version: the difference is real but narrower than the names suggest, and for most people the deciding factor is one specific symptom.
What both toothpastes share
Start with the common ground, because it is most of the product.
Both are fluoride-free. Both are built around microRepair®, which is Biorepair's patented zinc-substituted carbonate hydroxyapatite. Hydroxyapatite is the mineral that makes up the great majority of tooth enamel — approximately 96 to 97 per cent by weight — which is why the approach is described as biomimetic: it supplies the tooth surface with the same material family the tooth is built from, rather than a substitute chemistry.
Both come in 75ml tubes, both are £6.99 at Onyx, and both are intended for ordinary twice-daily brushing. Neither is a treatment product you use in courses.
Biorepair's description of how microRepair® works is consistent across the range: during brushing, the particles deposit enamel-like calcium and phosphate onto the tooth surface, bind to microscopic enamel irregularities, and support remineralisation of early surface mineral loss. With regular twice-daily use the manufacturer describes this as forming a temporary biomimetic coating over enamel-surface micro-scratches.
That description has a hard boundary and we will state it plainly: this is not restoration of lost tooth structure. It does not fill a cavity, mend a fracture or repair a chip. Those need a dentist.

The concentration gap — 24 per cent against 20 per cent — is the headline difference, and it is the higher of the two figures Biorepair publishes for this range. It is a real difference, but note the scale: both are high-concentration formulas by the standards of the hydroxyapatite market, where products at low single-digit percentages are common. The choice here is between "high" and "slightly higher", not between "active" and "token".
Why the sensitivity version exists
Dentine hypersensitivity happens when dentine loses its covering — through gum recession, enamel wear, abrasion from heavy brushing, or grinding — and the microscopic tubules running through it can transmit cold, sweetness and touch towards the nerve. Occluding those tubules is one of the two standard approaches to managing it.
Hydroxyapatite has a good case here. The 2023 systematic review and meta-analysis by Limeback, Enax and Meyer in Biomimetics pooled 44 human clinical trials and found hydroxyapatite products significantly reduced dentine hypersensitivity compared with placebo, and compared with fluoride, in the pooled analysis. Against other established desensitisers the difference was a non-significant tendency.
Biorepair additionally publishes a manufacturer claim of sensitivity relief in 30 seconds, based on a study of 100 participants assessed with the Schiff Air Index and a visual analogue scale, reported as average results. We always quote it with that qualifier attached. An average across 100 people describes a group, not a promise to an individual, and results vary.
Why the everyday version exists
Total Protection is the general-purpose tube. If you do not have a sensitivity complaint, you are not gaining anything by paying for a sensitivity-focused formula — and since both cost the same at Onyx, this is genuinely a fit decision rather than a value one.
For everyday enamel-surface care, 20% microRepair® is a substantial concentration, and the mechanism is identical. This is the tube we point most people to when they simply want a fluoride-free toothpaste that has something real in it.
How to choose in one minute
Choose Sensitive Teeth if
- Cold drinks, cold air or ice cream make you wince
- Your dentist has mentioned gum recession or exposed root surfaces
- You have noticeable wear, or you know you brush firmly
- You grind or clench
- You are unsure — the higher concentration costs the same, so it is the safe default
Choose Total Protection if
- You have no particular sensitivity and simply want fluoride-free daily care
- You are buying for a household where nobody has a specific complaint
- You prefer the everyday formulation and taste
Since the prices are identical, there is no penalty for starting with Sensitive Teeth and moving to Total Protection later, or the reverse. Many households keep one of each.
Getting the most out of either one
Three habits make more difference than the four percentage points between the two tubes.
Spit, do not rinse. Rinsing with water immediately after brushing washes the active straight down the sink. NHS guidance makes the same point about fluoride toothpaste, and the logic is identical here: leave the paste in contact with the teeth.
Twice a day, about two minutes, every day. Remineralisation is cumulative. Occasional use of a good toothpaste is worse than consistent use of an adequate one.
Ease off the pressure. Hard brushing contributes to the wear that causes sensitivity in the first place. If you cannot tell how hard you are pressing — and most people cannot — a brush with pressure feedback such as the Oral-B iO9 will tell you honestly.

Common questions
Can I use them together?
Yes, and some people do — Sensitive Teeth at night when the mouth is left undisturbed for longest, Total Protection in the morning. There is no interaction to worry about; both are the same technology at slightly different concentrations. That said, it is not necessary, and if you are managing sensitivity you will get a clearer read on whether it is working by using one formula consistently for six to eight weeks.
Are they safe for children?
These are adult formulations and are not marketed as children's toothpaste. UK guidance for children is specific about fluoride levels by age, and children's toothpaste choice is a conversation to have with your own dentist rather than one to settle from a product page. If you are considering a fluoride-free option for a child, raise it at their next appointment.
Will either whiten my teeth?
We do not make whitening claims for either product, and you should be sceptical of anyone who does without stating what was measured. Any toothpaste can lift some surface staining through ordinary mechanical cleaning. That is not the same as changing the underlying shade of a tooth, which is a different process entirely.
Is zinc hydroxyapatite different from nano-hydroxyapatite?
They are related but not identical. Both are hydroxyapatite; microRepair® is described by Biorepair as a zinc-substituted carbonate hydroxyapatite, meaning zinc ions are incorporated into the mineral structure. "Nano-hydroxyapatite" is a description of particle size rather than a different mineral. When you compare products, the concentration and the manufacturer's own description are more informative than the prefix.
How long should a tube last?
For one adult brushing twice daily with a pea-sized amount, a 75ml tube usually runs four to six weeks. If yours is emptying in a fortnight, you are almost certainly using far more paste than you need — a pea is genuinely enough, and using more does not improve the result.
